U-PASS Error Reporting Form Use this form to report a non-functioning U-PASS. If you need your first U-PASS or want to reactivate your U-PASS, please fill out our U-PASS Activation Request form first. If you have already done so and are encountering issues please fill out this form. If you lost or damaged your U-PASS, you will need to purchase a replacement through our website for $25. Otherwise, you can use this form to report an error. U-PASS Error Reporting FormPlease check the MCTS section of your Transportation Services Account for the following questions. It is located at the bottom of the account management page. Please note you cannot change the information on that page.
