U-PASS Error Reporting Form Use this form to report a non-functioning U-PASS. If you need your first U-PASS or want to reactivate your U-PASS, please fill out our U-PASS Activation Request form first. If you have already done so and are encountering issues please fill out this form. If you lost or damaged your U-PASS, you will need to purchase a replacement through our website for $25. Otherwise, you can use this form to report an error. If your U-PASS was stolen, please contact Transportation Services through firstname.lastname@example.org or by calling 414-229-4000. U-PASS Error Reporting FormPlease check the MCTS section of your Transportation Services Account for the following questions. It is located at the bottom of the account management page. Please note you cannot change the information on that page. You must return to this form to get assistance.Do we have the right U-PASS on file?*Check the MCTS Card Number in your account and compare it with the number on the lower left of the backside of your card. Is your U-PASS listed as active?Check the Is My U-PASS Active? field in your account and enter in the corresponding value below. Please note that new U-PASSes may take up to 96 hours for MCTS to activate your card on all of its buses after you receive a new U-PASS or request reactivation of your old U-PASS. Your U-PASS is listed as active according to the records we sent over to MCTS. Usually, it is just an issue with the farebox, where it misreads the card or accidentally reads it twice. If you show your student ID to the driver, you should be able to ride. Try it again after 5 minutes to determine if this is the case. If the bus driver is giving you a hard time, call the customer service number on the back of the card because they have a list of all active cards.
