Community Health and Wellness Centers
For over 30 years the UW-Milwaukee School of Nursing has been working alongside residents in Milwaukee to promote healthy communities. Health information and care coordination services along with varied and engaging health promotion programs are the hallmark of the nurse-led partnership sites.
Nurse clinicians partner with program managers at the centers to coordinate health education and prevention services for guests and residents from the surrounding area. Many UWM School of Nursing and other health professional students benefit from having their community health clinical experiences at the House of Peace and Silver Spring Community Nursing Centers.
UWM Community Health and Wellness Center – House of Peace
Located within the Capuchin Community Service House of Peace site in the Walnut Way neighborhood, the Center staff develop strong and lasting relationships with clients and are active advocates for their health and well-being.
A range of services provided at the House of Peace Community Nursing Center including:
- Health information and referral to community resources
- Health screening, education, and monitoring for chronic conditions
- Health promotion classes
- Gift closet which provides services and supplies to women who have had breast cancer

UWM Community Health & Wellness Center- Silver Spring Neighborhood Center
Located within the heart of Westlawn, Wisconsin’s largest subsidized housing development and within the walls of Silver Spring Neighborhood Center, the Center staff maintain strong partnerships and advocate for community health and well-being.
A range of health services provided at the Silver Spring Community Nursing Center:
- Health information and referral to community resources
- Health screening, education, and monitoring for chronic conditions
- Health care education and guidance
- Collaborative activities with community partners focused on population health
