Take a Course a Third Time

Complete this form to request permission to take a course for a third time. Enter a statement of rationale addressing the reason for academic difficulty in your first two attempts of the course and the steps you will take for a successful outcome if allowed to repeat.

Upload the following item before you submit your request:

A Registration Change Form (PDF) signed by the academic department (if adding course after the semester add deadline)

Upon receipt your advisor will review and if approved, enter the necessary permissions in PAWS. The appeal decision will be communicated to you via your UWM email address.

NOTE: Follow the procedure below except if seeking permission for a third attempt of ENGLISH 100, 101, or 102. Repeats of English composition are handled by the Writing Program Director, Shevaun Watson at watsonse@uwm.edu.
