Assumption of Risks

I understand that the act of participating in any activity, by its very nature, carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. The specific risks of participation in Peck School of the Arts (PSOA) activities vary from one activity to another, I understand that I have the right to request information regarding these risks, and I agree to report any injuries or incidents relating to my participation. I acknowledge that I have been advised to have health and accident insurance in effect and that no such coverage is provided for me by the Board of Regents of the University of Wisconsin System, UWM or the State of Wisconsin. Hold Harmless, Indemnity and Release

In exchange for permission for my child to engage in and to continue to voluntarily participate in rehearsal and performance activities at UWM, today and on all future dates, I, for myself, my heirs, personal representatives or assigns, agree to defend, hold harmless, indemnify and release the Board of Regents of the University of Wisconsin System, UWM, and their officers, employees, agents, and volunteers, from and against any and all claims, demands, actions, or causes of action of any sort on account of damage to personal property, or personal injury, or death which may result from my participation in the above- identified activities. This release includes claims based on the negligence of the Board of Regents of the University of Wisconsin System, UWM, and/or their officers, employees, agents, and volunteers, but expressly does not include claims based on their intentional misconduct or recklessness. I understand that by agreeing to this provision I am releasing claims and giving up substantial rights, including my right to sue.

I acknowledge that in the case of injury or illness, I will be contacted at the phone numbers I’ve provided. In the event I cannot be reached, I grant authorization and consent for UWM to administer general first aid treatment for minor injuries or illnesses. If the injury or illness is severe, I authorize UWM to seek professional emergency personnel to attend/ transport/ and treat my child and to issue consent for any medical care deemed advisable by a licensed medical professional or institution.

Minors Protection

Due to new, stricter minors’ protection guidelines and supervision ratios, we are required to always maintain supervision of the participants during the day. If you do not want your student to be able to leave the event unsupervised (i.e. to use the restroom) please contact the appropriate programs manager listed above.

Release for Use of Photos and Likeness

I understand that the University of Wisconsin-Milwaukee (“UWM”) may take photographs, video, audiotape and other image and sound-based media of the campus, (including inside its buildings and the use of drones), and its employees, students, and visitors (collectively, the “Images”). UWM may wish to use such images for educational, promotional, advertising, and other purposes, including, but not limited to printed publications, on its website, and submission to conferences/festivals/screenings. Therefore, I hereby freely and voluntarily consent to the use and publication of my child’s name, participation, picture, and/or likeness by UWM and/or its employees and/or agents for any and all purposes including, but not limited to, educational, promotional, advertising, grant applications and trade, through any medium or format as technology may now or in the future allow, including, but not limited to, videotape, audiotape, webcasting, podcasting, film, photograph, television, radio, digital, internet, theatre, or exhibition, at any time from this date forward. I further waive any claims against UWM and/or its employees and/or agents based upon or related to its use or publication of my child’s likeness, voice, participation, and/or picture. I freely give this authorization without expectation of compensation.
