Individual Travel Excuse Request This form shall be used by individuals to request permission to travel separately from their club to/from a given event or competition. Submission of this form does not guarantee the request will be granted, and sport club members will need confirmation from their respective Coordinator before they may be excused from travelling with their club. Granted requests will only apply to the individual who submits the form, and all other club/team members are still subject to all Sport Club Program policy regarding travel.
