Event Access Request Form

The event door access form is designed to regulate and monitor entry into the Klotsche Center pavilion and corresponding areas during an event. This form serves the purposes of coordinating security and managing attendance in Klotsche Center spaces. Below, please provide detailed hours of requested access to designated Klotsche center spaces.

Pavilion Exterior Doors
The exterior doors are the entry points to the pavilion from the outside and parking structures. These doors serve as the primary access points for attendees and staff entering or exiting the pavilion during the event.

Athletic Suite Doors
The athletic suite doors are located within the Klotsche pavilion and provide access to approved personnel (coaches, players, admin) to the office spaces of UWM athletic department.

Event Doors
Event doors will be the physical entry point of the event where access will be determined by the organizer of the event. These doors will serve as the gateway for attendees, staff, and vendors.
