This service is available on a walk-in basis in Garland Hall 138 between 10:00 am and 4:00 pm Monday through Friday.

The Power of Attorney forms need to be notarized in order to be valid. Notaries can be found at banks and credit unions, the University Legal Clinic, and there are several on staff in Chapman Hall. Please plan to go during normal business hours, and realize a notary may not always be available. If possible, try to set up an appointment to ensure a notary will be on site.
Once enrolled in the insurance, you will receive information regarding access to the CISI health insurance online portal. There you will be able to:
- View/print/e-mail your ID card, coverage brochure, consulate letter, and claim form
- Purchase extra insurance for a period of personal travel
- View/update your online account profile information
- Search for hospitals and clinics overseas
- Find valuable travel-related information
Seek medical assistance for any incident that you think may require service. CISI will not cover expenses for follow-up medical service in the United States if an incident report was not filed while abroad.
Many hospitals, clinics, and doctors abroad will not accept CISI insurance at the time of service. This means that you (or the sick or injured student) will have to pay for the medical care, then later submit a claim form for reimbursement (paying for medical service fees is not the responsibility of UWM or The Center for International Education). To ensure that you are reimbursed, it is very important that you keep all medical service receipts.
Travel Registration and International Health Insurance
Here at UWM’s Center for International Education, we want to assure your time abroad is as safe and enjoyable as possible. To ensure this, it is important you register with CIE.
This registration process will collect important information about your travel, as well as essential information like your passport details and emergency contact information – both here in the U.S. and abroad.
Student Travel:
Registration should be completed at least three weeks prior to departure (i.e.: if you depart June 1, you must register by May 11) to ensure proper processing. Any registration requests submitted within three weeks of departure could be subject to a $50 late registration fee. If applying within five business days of departure, a rush fee of $50 will be applied in addition to the late application fee.

Processing of registrations and insurance is not done same day. Please plan accordingly.
Students

Please note that students are charged a $100 International Activities fee that includes the cost of insurance for up to four weeks and emergency support, as well as access to all of the Center for International Education’s services. Additionally, it allows the Center for International Education to enroll students in the State of Wisconsin mandated international health insurance, administered by Cultural Insurance Services International (CISI). If international travel is longer than one month in duration, the student will be charged an additional fee to extend coverage.
This insurance is specifically designed for the international context and will provide far more robust and comprehensive coverage than any policy you would normally have from a U.S.-based policy.
Coverage is available in one-month increments, at the rate of $35/month for 2019-2020. (for example, if you are going for 5 weeks, you will be responsible for paying 2 months of insurance). These charges will be posted to your PAWS account unless other payment arrangements are made.
Faculty/Staff
By registering your travel you will also be able to request CISI international health insurance enrollment coverage. CISI is not travel insurance, so it may not necessarily be used to cover the cost of canceled travel or lost luggage. This insurance is meant to supplement, not replace, travelers’ regular insurance plans. The cost of the insurance can be paid for by check or money order to the Center for International Education or invoiced to a department or unit on campus.
Please note it is also possible to have someone register on your behalf as a proxy.

If you are unsure about your eligibility you should e-mail travel-cie@uwm.edu with the information indicated below no later than one week before your scheduled international travel if traveling alone or with other faculty/staff, and no later than three weeks before departure if traveling with students.

Following receipt of registrations, the Enrollment Coordinator will review the information submitted to determine eligibility and follow up on missing materials. Review criteria include, but are not limited to, having a clear connection to UWM travel-related business, and whether or not the university will be able to ensure adherence to legal and risk management principles. Once enrolled in the insurance, they will then be contacted by the Enrollment Coordinator with CISI materials and about payment for coverage.
2019-2020 Rates:
- One week: $25
- Two weeks: $33
- Three weeks: $41
- Four weeks: $50
- + $35 for each additional month

Proxy Travel Registration
