Dear UWM Faculty and Staff,
This is a reminder inviting you to participate in an important suicide prevention survey. You are among a small group of UWM faculty and staff members who have been randomly selected to participate in the UWM Suicide Prevention Project Awareness and Knowledge Survey (USPPAKS). UWM received a grant in 2012 from the federal Substance Abuse and Mental Health Services Administration (SAMHSA) to implement a comprehensive campus suicide prevention program for our campus. Your responses to this survey will be used to inform and improve campus programs to prevent suicide attempts and deaths among UWM students. Even though you may have participated in this survey about one year ago, we would appreciate your responses to this survey again this year.
If you have not yet responded to the survey, there is still time to participate and your response is very important to us! Please complete the survey. This survey should take just 5-10 minutes to complete.
Please note that all responses are anonymous and will be used for program design and improvement purposes, only.
