Directions to Fort Memorial Hospital

Address: 611 Sherman Ave E, Fort Atkinson, WI 53538 (approximately 63 minutes from the UWM campus)

Instructions:

Enter the hospital through the main entrance on Armenia Street.  Go to the Outpatient Lab, directly to the right when you walk in, and let them know that you are there for the UWM Student Lab Tour.

If you have signed up for this tour but know in advance that you are unable to attend, please email Brad at bkdepons@uwm.edu.  If it is the day of the tour and you are running late or are not able to attend, please call or text Brad at 414-803-7965 to let him know.
