The purpose of this form is to enable campus community members to request support to intentionally address challenges and opportunities in their departments/units/programs that will lead to increased equity and belonging. When you submit a request, a member of the Division of Community Empowerment & Institutional Inclusivity leadership team will reach out to you to discuss your needs and interests.
