Ascension Health transitioned their laboratory operations to Labcorp in 2022. At this point we have been advised to continue with the normal onboarding process used by Ascension Health, but this may change. Any changes to the onboarding requirements will be communicated to students as soon as possible.

Follow the instructions below to complete the online orientation. The confidentiality statement must be printed out and signed, and you should forwarded this to Brad De Pons. Forms may be submitted to Brad via email at bkdepons@uwm.edu, or paper copies can be dropped off. If submitting via email, please submit good quality scans of each document.
Please Note: Use Google Chrome or Mozilla Firefox browsers on a desktop or laptop computer as some of these orientation modules are not supported by Internet Explorer and mobile devices.
