School Librarianship Confirmation Form

To pursue the School Library Media Specialist License, please complete the form below. You must first be admitted to UWM as a student in our MLIS program or our post-bacc school library certificate program in order to complete the form.

  • Your 9-digit UWM Student ID Number is issued after you have been admitted to UWM. You must first be admitted to UWM as a Graduate Student or Undergraduate Certificate Student before completing this form.
  • Please upload your background check report from Viewpoint Screening, dated within the past 6 months. Visit this link for Viewpoint Screening Instructions.

    • Please upload your undergraduate transcripts, (official or unofficial), below. If applicable, please upload graduate transcripts as well. Based on your experience, credits can be waived.
      Note: This is a post-baccalaureate program – a baccalaureate degree from an accredited college or university is required.
