The Well Wisconsin Program offers tools and resources to help you achieve a healthier lifestyle.

The three steps to earn the Well Wisconsin Incentive can be completed from home.

Well Wisconsin Incentive

The Well Wisconsin Incentive program is a voluntary program available to employees, retirees and spouses enrolled in the State of Wisconsin Group Health Insurance Program, excluding Medicare Advantage participants who have incentives available through their healthplan.

The Well Wisconsin Incentive is a $150 Well Wisconsin gift card that will automatically be issued to eligible participants upon completing the three steps described below. The deadline for completing all program requirements is October 9, 2020.

Your Three Steps:

  1. Complete Your Health Screening From Home.

    Download the fillable Health Care Provider Form (HCPF).

    Locate blood pressure and weight numbers from your most recent physician visit.

    Enter those values on the HCPF.

    Upload your HCPF using one of the methods listed on page 1 of the form.

  2. Complete Your Health Assesment From Home.

    Log into your StayWell portal (wellwisconsin-staywell.com) and follow the steps to complete your health assessment.

  3. Complete Your Well-Being Activity From Home.

    Complete a well-being activity, such as the Million Steps Challenge, or connecting with a health coach. For the full list of well-being activities, please see the My Incentives tab in your StayWell portal (wellwisconsin-staywell.com).

All wellness incentives paid to participants are considered taxable income to the group health plan subscriber and are reported to their employer, who will issue a W-2. In some cases, the Wisconsin Retirement System acts as the employer. Retirees, continuants and their spouses will have some taxes withheld from the incentive amount earned.

Well Wisconsin Incentive

Employees and their spouses who are enrolled in the State of Wisconsin Group Health Insurance Program are eligible to earn the $150* Well Wisconsin Incentive by completing three easy steps by October 9, 2020.

Your Three Steps:

  1. Have a health screening in 2020 and submit your results to StayWell.**
  2. Take the 10-minute StayWell on-line health assessment.
  3. Complete one well-being activity through your StayWell portal.

The deadline for completing all program requirements is October 9, 2020.

* Those with a postal address outside the US are not eligible to receive the incentive. The incentive is treated as taxable income.

** Individual health information will never be shared with the employer.

Diabetes Management

If you have diabetes, you can take part in a StayWell disease management program with a Certified Nurse Consultant. In exchange, you will receive lower prescription drug copays. There is no cost to participate in the program.

How it Works

  1. Enroll in the program and meet with your dedicated nurse consultant over the phone.
  2. Your nurse consultant will work with you to help you reach your health goals. All contact with your nurse will remain confidential.
  3. If you keep talking to your nurse consultant on a monthly basis, your copay for anti-diabetic drugs will be lowered.
