The Custodian is responsible for maintaining a current, detailed inventory of all specimens in their care, which must be submitted through I-Manager when changes occur.
Required Documentation
As described in UWM Policy, all human anatomical specimen inventories must include the following required documentation:
Campus Details
- All laboratory & storage locations for specimens while on campus
Acquisition Details
- Origin
- Contractual obligations / terms of procurement
- Invoice / receipt & PO number
- Materials retention agreements (if long-term retention)
Specimen Details
- ID number
- Arrival date
- Return / disposition date
Inventory Template
A UW working group developed a template to use across all UW campuses. Use this template to document specimen holdings. Please do not add, delete, or reorder columns. This enables inventories from multiple custodians and/or multiple institutions to be combined into a full list of UWM or UW holdings. You can, however, hide columns that are not applicable.
Acquisition
Human Anatomical Specimens may only be acquired from ethically operated body donor programs. The source organization must have clear methods for ethically accepting body donations and should provide cadavers and other specimens for research or educational purposes only.
Purchases of human anatomical specimens may only be made through Purchase Orders (POs). They may not be purchased using a P-Card.
Only approved Custodians may acquire new specimens. All others must be approved to be Custodians before initiating any acquisition.
Approved Donor Sources
UWM maintains a list of approved donor sources.
If the donor source you wish to use is not on the list, it must be vetted and approved. Contact uwm-haso@uwm.edu for assistance. We will guide you through the process.
