Return to Year 4 Summer Session

Any changes or additions to site-specific onboarding paperwork are typically made in late summer/early fall.  The instructions below were last updated in October of 2023 and are current for the 2023-2024 clinical rotation.

Review and sign the Student/Faculty Certification Attachment to Exhibit A form and forward to Brad.  Forms may be submitted to Brad via email at bkdepons@uwm.edu, or paper copies can be dropped off.  If submitting via email, please submit good quality scans of each document. You will sign in the Student/Faculty spot, print your full name, and date the form.
