Microsoft Forms is a web-based survey and data collection tool that is part of the Microsoft 365 suite licensed to students, faculty, and staff at the University of Wisconsin–Milwaukee (UWM). It allows users to easily create forms, quizzes, and surveys to gather feedback, assess learning, or collect information, with built-in features such as customizable questions, branching logic, and automatic grading for quizzes. Responses are collected in real time and can be viewed through summary dashboards or exported to Excel for further analysis. Microsoft Forms integrates with other Microsoft 365 applications such as Teams, SharePoint, and OneDrive, making it simple to share forms and collaborate with others. As part of UWM’s Microsoft 365 environment, Forms provides a secure, accessible, and user-friendly way to collect and analyze data from any device with an internet connection.
