Social Work Licensure Request Form

Please fill out this form if you are looking to submit paperwork for social work licensure. This form is for graduates of the Bachelor of Science in Social Work or Master of Social Work program at UW-Milwaukee.

BSW students should send the 1978 form to the Registrar’s Office at regoff@uwm.edu for approval and processing. MSW students should send the MSW 1978 form to the Graduate School at gradschool-staff@uwm.edu for approval and processing.
