For UW-Milwaukee Police Department (UWMPD) record requests, fill out the UWMPD request form. You may also email police-records@uwm.edu.

UW-Milwaukee follows Wisconsin’s Public Records Law (Wis. Stat. §§ 19.31–19.39). We will answer your request as fast as we can.

Requesting Student Conduct Records/Dean’s Certification

Some schools or programs ask to see your UW-Milwaukee conduct record before they admit or enroll you. Would you like UW-Milwaukee to fill out a Dean’s Certification or Letter for you? Fill out the Student Conduct Records/Dean’s Certification Request Form.
