Regarding the US Department of Education
We are aware of the new Executive Order related to the U.S. Department of Education. We hope that this executive order will not affect federal student financial aid for the current semester and continue to diligently seek more information. We will continue to closely monitor developments and remain prepared to navigate any changes. If you have concerns about your financial aid, please contact the UWM Student Financial Service Center.

I Filed a 1040 Tax Return

Provide 2023 tax information for yourself, spouse (if applicable), and parent(s) (if applicable) using one of the following methods below:

  1. Submit a signed copy of 2023 federal tax return (1040), including schedules 1, 2 and 3 if applicable.
  2. Request a 2023 IRS tax return transcript using one of the following:
    • Online: Get Transcript | Internal Revenue Service (irs.gov)
      • Select “Get Transcript by Online” or “Get Transcript by Mail” Please note, some individuals may not be able to obtain online.
      • Complete the required information.
      • Select “Return Transcript” for the 2023 tax year. Make sure to request “Return Transcript” and NOT the “Account Transcript”.
      • Be sure to list student name and ID# on all transcripts as the IRS does not print identifying information on the transcript.

    • Phone: Call 800-908-9946
    • Mail: Complete and submit IRS Form 4506-T and mail to the IRS. Do not return 4506-T forms to the Student Financial Service Center.

I Have an Amended Tax Return

If an individual filed an amended IRS Federal Income Tax Return for tax year 2023, provide both items:

  • Signed copy of the 2023 original federal tax return (1040),), including schedules 1, 2 and 3 if applicable or a 2023 IRS Tax Return Transcript.
  • A signed copy of the 2023 “Amended U.S. Individual Income Tax Return” (IRS Form 1040X) and any applicable amended schedules, that was filed with the IRS.

Sometimes the IRS makes adjustments to taxes on a tax filer’s behalf. If that is the case, you may not have a copy of a 1040X. If you do not have a copy of your 1040X, for whatever reason, you will need to provide a Record of Account Transcript that can be obtained at IRS.gov or by completing form 4506-T and submitting it to the IRS. Do not return 4506-T forms to the Student Financial Service Center.

I Experienced IRS Identity Theft

If an individual was a victim of IRS Identity Theft, provide both items:

  • Signed copy of the 2023 original federal tax return (1040),), including schedules 1, 2 and 3 if applicable or (TRDBV) transcript for 2023.
  • Submit an IRS 4674C letter (a letter from the IRS acknowledging the identity theft) or provide a signed and dated statement by the tax filer (by both parents, if married) indicating the individual(s) was a victim of IRS tax-related identity theft and the IRS is aware of the tax-related identity theft.

Example: I, (insert your name), was a victim of IRS tax-related identity theft and the IRS has been made aware of this tax-related identity theft.

I Have a Filing Extension

An individual who is required to file a 2023 IRS income tax return and has been granted a filing extension by the IRS beyond the automatic six-month extension for tax year 2023, must provide:

  • A copy of the IRS’s approval of an extension beyond the automatic six-month (October 15, 2024) extension if the individual requested an additional extension of the filing time for the tax year 2023;
  • A copy of IRS Form W–2 for each source of employment income received for the 2023 tax year; and
  • A signed and dated statement (by both parents, if married) certifying the estimated amount of the individual’s Adjusted Gross Income (AGI) and the U.S. income tax paid for the 2023 tax year. For the AGI this would be the amount on line 11 on the IRS Form 1040. For taxes paid, this would be the amount on line 24 on IRS Form 1040.

Example: For the 2023 tax year, I (insert name) and my spouse (if married, insert name) have been granted an extension from the IRS. Listed below are the best estimates at this time for the following:

I Filed a Non-IRS Tax Return

If an individual filed or will file a 2023 income tax return with Puerto Rico, another U.S. territory (e.g., Guam, American Samoa, the U.S. Virgin Islands, the Northern Marianas Islands), or with a foreign country (in which a tax transcript is provided free of charge), please provide a signed copy of that 2023 income tax return. You also need to convert amounts to U.S. Currency and detail the equivalent line of the following information:
