Roommate or Suitemate Acknowledgement of Assistance Animal

Once a student is approved for an assistance animal or a service animal in training, they must complete the community acknowledgement form. Download the form, add the information and obtain the necessary signatures. Upload the signed form to your Assistance Animal or Service Animal in Training Expectations Web Form.
