Documentation is essential for the Accessibility Resource Center (ARC) to develop an effective accommodation plan for each student. The documentation form, completed by your provider, can serve as a source of primary documentation for academic accommodations. When applying only for Housing Accommodation, go to page 5 of the documentation form and complete the Supplemental Disability Documentation for University Housing Accommodation Requests section.
You may also complete the Authorization for Release of Confidential Information form (PDF), requesting your provider or health facility to send documentation directly to ARC.
Academic accommodations are available to all UWM students, whether full-time or part-time, at any stage of their college career. These may include academic flexibility, notetaking services and tools, alternative formats, assistive technology, and more.
Students residing on the UWM campus may request housing accommodations, including dietary needs, assistance or service animals, dorm room modifications, and emergency evacuation arrangements.

UWM dual enrollment students must work with the Accessibility Resource Center (ARC) to establish an Accommodation Plan. Instructors will coordinate with students or ARC to provide accommodations.
