This series of four web-based modules was created for CCC providers, in collaboration with the Wisconsin Department of Health Services. This series is intended for both new providers and more experienced providers who are looking to improve their knowledge about comprehensive and important skills, such as early childhood development, behavioral health, and the role of care coordination in Wisconsin Medicaid. These are fully asynchronous and include resources and interactive activities, including a certificate for providers who complete the module. The goal of these modules is professional development, and all Medicaid providers must follow Medicaid policy.

Module 1: Care Coordination with Children and Families

Module 1 provides a foundational understanding of the CCC benefit for children and families and equips providers with practical strategies for coordinating care.

This module is 35 minutes long.

Module 2: Early Childhood Development

Module 2 is about the basics of early childhood development including milestones, timelines, and evidence-based programs available to children.

This module is 80 minutes long.

Module 3: Coordinating Behavioral Health Services for Children and Families

Module 3 explores available behavioral health services and how to recommend appropriate resources. You will also gain the skills needed to effectively coordinate behavioral health support for members and their families.

This module is 50 minutes long.

Module 4: Engaging Partners and Aligning Care in CCC 

Module 4 discusses the skills needed to engage natural supports, community providers, and system partners in collaborative care coordination for children and families.

This module is 35 minutes long.
