I am the owner and copyright holder of the following photo(s)/image(s) (collectively, the “Images”)

I understand that the University of Wisconsin-Milwaukee (“UWM”) wishes to use and/or publish the Images for educational, promotional, advertising, and other purposes, in any media whatsoever, including, but not limited to printed publications and on its website (collectively, the “Authorized Uses”).

I further understand that UWM, where feasible, will credit me as follows to the extent that it publishes the Images

In consideration for the publicity I may obtain through UWM’s use of the Images, by signing below, I grant UWM and the Board of Regents of the Universities of Wisconsin an irrevocable, worldwide, license to use the Images for the Authorized Uses. I understand that I will retain the copyright in the Images. I also freely and voluntarily consent to the use and publication of my name in connection with the Images.

I also waive any right I may have to inspect and/or approve use of the Images or any reproductions thereof.
