  • Installing Voice Mailbox(es)

    Installing means a new, basic voice mailbox will be created for the requested phone number(s).

    NOTE: If features other than a new, basic voice mailbox are needed (e.g.: call handler, alternate extension, phantom mailbox, etc.), select Additional Information and provide the details.
  • Provide the phone number(s) you wish to have a new, basic voice mailbox(s) installed on.
  • Removing Voice Mailbox(es)

    Removing voice mailbox(es) means all voice mailboxes and any associated voice mail features will be removed entirely; all saved data will be lost.

    NOTE: There is no charge for this service.
  • Provide the phone number(s) you wish to have all voice mailbox(s) removed from.
  • Adding Phone Number(s)

    Adding means you want to put existing phone number(s) on additional jack(s) as well as remaining on their current jack(s).

  • Disconnecting Phone Number(s)

  • Funding String Change(s)

    Requesting to change the funding string that pays for phone number(s) from the existing funding string to a different funding string. If a new funding string is needed, first request a new funding string request from the UITS Accounting Department then complete this form.

  • Toll-Free Number

    Requesting a toll-free number means our vendor will supply a random, unique 8xx number that will terminate incoming calls to an existing phone number (this can be any phone number type, including a Teams phone number; if a new phone number is needing to be installed, please, request that service above).

  • NOC Technician Supplies

    For NOC Technician use only. Supply purchasing is complex. We must always start by searching ShopUW+ and purchasing their if found. If not found at ShopUW+, we must purchase through any active POs. If no POs are active/applicable, we must create a requisition in ShopUW+. Follow-up questions will occur.
