COVID-19 Symptom Monitor

COVID-19 Symptom Monitor

  • Note: we do not save or collect any data from this form.

    • Fever (temperature at or above 100.4°F/38°C)
    • Shortness of breath or difficulty breathing
    • Cough that is unexplained
    • Fatigue that is unexplained
    • Nausea or vomiting
    • Diarrhea (multiple episodes of loose or watery stools)
    • Chills with or without shaking
    • Muscle pain that is unexplained
    • Runny nose or nasal congestion (new or worsening from baseline)
    • Sore throat
    • Loss of taste or smell
    • Headache that is unexplained (different than typical headaches)
  • Guidance for Students

    • Do not come to campus.
    • If you currently live in a university residence hall, remain there and in your room.
    • Contact Norris Health Center at 414-229-4716 or your primary care provider to review your symptoms and schedule a COVID-19 test if indicated.
    • Remain in your residence hall room or away from campus until your test result is negative or a healthcare provider approves going to campus.
    • Refer to additional guidance from the Centers for Disease Control and Prevention.
  • Guidance for Employees

    • Do not come to the workplace.
    • Contact your healthcare provider for a COVID-19 test or approval to be at the workplace.
    • Notify your supervisor you will be away from on-site work.
    • For leave resources and/or for sharing medical information, contact your HR Business Partner (HRBP) or Prep, who as needed, may contact an appropriate Central HR Specialist.
    • Refer to this additional guidance from the Centers for Disease Control and Prevention.
  • Guidance for Contractors

    • Do not come to UW-Milwaukee facilities.
    • Contact your healthcare provider for a COVID-19 test or approval to be on campus.
    • Do not return to the campus workplace until your test result is negative or your healthcare provider advises that you may return to work.
    • Refer to this additional guidance from the Centers for Disease Control and Prevention.
  • Guidance for Visitors

    • Do not come to campus.
    • Contact your healthcare provider for a COVID-19 test or approval to be on campus.
    • Do not come to campus until your test result is negative or your healthcare provider advises that you are safe to go to campus.
    • Refer to this additional guidance from the Centers for Disease Control and Prevention.
  • Start Over
  • Close contact is considered as being within 6 feet of another person for 15 minutes or longer. If your close contact is due to providing clinical care for COVID-positive patients while following health system appropriate protocols, answer No.
  • Guidance for Students

    • You may come to campus. You must wear a mask at all times for 10 days from last date of close contact.
    • You must monitor for symptoms until 10 days after last close contact.
    • You are advised to get tested after at least 5 full days from the last instance of close contact with infected person. Note: The date of the last instance of close contact is considered Day 0.
    • If you have been in contact with public health officials, such as contact tracers, follow their guidance.
    • Refer to additional guidance from the Wisconsin Department of Health Services and the Centers for Disease Control and Prevention.
  • Guidance for Employees

    • You may come to the workplace. You must wear a mask at all times for 10 days from last date of close contact.
    • You must monitor for symptoms until 10 days after last close contact.

    • If you currently live in a university residence hall, remain in your designated isolation space until your isolation period is over.
    • Follow the instructions of the provider who you received a COVID-19 test or diagnosis from in terms of returning to campus.
    • Contact the Dean of Students Office at 414-229-4632 if you have had contact with anyone or any location on campus.
    • Refer to this additional guidance from the Centers for Disease Control and Prevention.
  • Guidance for Employees

    • Do not come to the workplace until your isolation period is over.
    • Follow the instructions of the provider who you received the test or diagnosis from in terms of returning to campus.
    • Notify your supervisor you will be away from on-site work.
    • For leave resources and/or for sharing medical information, contact your HR Business Partner (HRBP) or Prep, who as needed, may contact an appropriate Central HR Specialist.
    • Refer to this additional guidance from the Centers for Disease Control and Prevention.

    • Follow the instructions of the provider who you received a COVID-19 test or diagnosis from in regard to returning to campus.
    • Notify your UWM contact person if you were on campus 48 hours prior to your positive test or symptoms.
    • Refer to this additional guidance from the Centers for Disease Control and Prevention.
