The Department of Human Resources utilizes the BP Logix e-workflow tool, also called Streamline, to route requests for review or approval outside of Workday. Please see the content areas below for the criteria and instructions on how to submit HR-related forms or requests.

1. Summer Appointment Request Form

This form is submitted to initiate a summer payment for an employee in a 9-Month (Academic Year) pay basis. Summer payments can occur as Summer Session (Teaching) or Summer Service (research or service). Each form should include all applicable summer appointments for the employee. Once submitted, it will be reviewed and approved by Human Resources prior to processing and contract generation. HR Business Partner will submit the Summer Excess Payment Request as warranted.

2. Recruitment Authorization and Intake

All hires or requests to recruit must be submitted and processed through this request form. Please be sure to include appropriate justification of the need for a hire, addressing the guidelines as provided below.

3. Compensation Approval Request

Form is used to obtain approval for compensation changes prior to entry into Workday. Included: Market, Parity, Performance (Base and One-Time), and Student Hires outside Range. Please be sure to include appropriate justification of the request per the appropriate policies and guidelines.

4. FMLA/WFMLA Request Process

This form is used to initiate the eligibility assessment process for the Federal and Wisconsin Family and Medical Leave Acts (FMLA). FMLA allows employees to take reasonable unpaid leave for certain family and medical reasons, and may be unpaid or used at the same time as employer-provided leave. Once submitted by an HR Business Partner, the FMLA Coordinator will work with the employee to review eligibility criteria and document the approval process.

5. Background Check Requests

The background check request form is utilized to request a Criminal Background Check and/or Misconduct Reference Check for volunteers as required by UW Policy. Once the form is submitted, respective checks will commence with the CBC Coordinator.
