Many of you are preparing for time away from campus, while others are getting ready to participate in commencement. Congratulations to our graduates \u2014 we\u2019re proud of you.<\/p>\n\n\n\n<p>As the semester winds down, we are sending a friendly reminder of some simple steps you can take for a smooth and healthy transition. Please take a moment to:<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-refill-your-prescriptions\">Refill your prescriptions<\/h2>\n\n\n\n<p>Request refills for any medications you\u2019ll need. Contact us early to avoid delays during finals week.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-complete-any-outstanding-visits-testing\">Complete any outstanding visits\/testing<\/h2>\n\n\n\n<p>If your SHAW provider has ordered labs or recommended a follow\u2011up visit, schedule those appointments now.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-pick-up-over-the-counter-essentials\">Pick up over\u2011the\u2011counter essentials<\/h2>\n\n\n\n<p>Stock up on commonly used items such as pain relievers, allergy medications, multivitamins, or other products available through our dispensary.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-pick-up-your-allergy-vials\">Pick up your allergy vials<\/h2>\n\n\n\n<p>If you receive allergy injections through SHAW\u2019s nursing staff, remember to arrange the pick-up of your vials before leaving campus. Please coordinate with your off-campus provider to continue your injections.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\" id=\"h-request-a-release-of-records-if-needed\">Request a Release of Records (if needed)<\/h2>\n\n\n\n<p>If a health care provider involved in your care requests your SHAW health records, please complete a Release of Information form. Forms are available on the <a href=\"https:\/\/uwm.medicatconnect.com\/\" class=\"uwm-c-link--external\">Patient Portal Welcome<\/a> for both medical and mental health records request.<\/p>\n\n\n\n<p>We\u2019re here to support your health year\u2011round. SHAW services are available during the summer by paying a summer use fee when not taking summer course(s). Visit <a href=\"https:\/\/uwm.edu\/wellness\/\">Student Health and Wellness Center<\/a> for additional information.
