The TTSTA Refugee Community-Centered Health Home (CCHH) toolkit was created to guide interested users in developing a refugee CCHH.

Intended to support refugee serving agencies, this toolkit will help you transform siloed infrastructures into across-system networks to improve access to primary health care. These efforts will facilitate refugee engagement in the US primary care system.
