Located within the Capuchin Community Service House of Peace site in the Walnut Way neighborhood, the House of Peace Community Nursing Center staff develop strong and lasting relationships with clients and are active advocates for their health and well-being.
Health information and care coordination services along with varied and engaging health promotion programs are the hallmark of the nurse-led partnership site.
Nurse clinicians partner with program managers at the House of Peace to coordinate health education and prevention services for House of Peace guests and residents from the surrounding area. Many UWM College of Nursing and other health professional students benefit from having their community health clinical experiences at the House of Peace Community Nursing Center.
A range of services provided at the House of Peace Community Nursing Center including:
- Health information and referral to community resources
- Health screening, education, and monitoring for chronic conditions
- Health promotion classes
- Gift closet for breast cancer survivors.
