To submit a title using this form you must:

• be employed by UWM;
• have your work published while a member of the UWM community;
• be a primary author, editor, translator, illustrator, or creator (referred to in this form as “Author”) of the published book, sound recording, motion picture recording, or musical score. (Articles, scholarly papers, chapter contributions, and similar publications are not included within the UWM Authors Collection.)
