The Department of Human Resources offers notary services. If you need a document notarized, please call (414) 229-4463, to make an appointment. Same-day appointments may not be available. There is no fee charged for notarization.<\/p>\n\n\n\n

Notarization Requirements<\/h2>\n\n\n\n

Please Note:<\/h3>\n\n\n\n

  1. Do not sign the documents in question until you are in the presence of the notary.<\/strong> This is because your signature must be witnessed by the notary.<\/li>\n\n\n\n
  2. A photo ID is required for notarization.<\/strong> If the documents require the signature of your spouse, or domestic partner, to be notarized, they must accompany you and bring their photo ID.<\/li>\n<\/ol>\n\n\n\n

    Also, please be aware that we are unable to make copies, certified or uncertified, of \u201cvital records,\u201d which include certificates of birth, death, divorce, annulment, marriage, etc. Copies of vital records must be obtained from their official custodian, such as a state or county office.<\/p>\n\n\n\n

    Furthermore, we will not certify that copies of documents (such as passports) are complete, full, true, and exact copies of the original document which they purport to reproduce.<\/p>\n\n\n\n
