Well Wisconsin Incentive Program 2018
Know Your Numbers!
As a part of the state’s Well Wisconsin program, employees and their spouses who are enrolled in the State of Wisconsin Group Health Insurance program are eligible to earn the $150* Well Wisconsin Incentive by completing three required steps through StayWell by October 19, 2018.
During the It’s Your Choice 2018 open enrollment period, Employee Trust Funds (ETF) had communicated plans to move the program incentive from a gift card to a State Group Health premium reduction in 2019 for active state employees. It has been determined this transition will not occur for 2019, and Well Wisconsin program participation in 2018 will NOT affect premium rates in 2019. Participants will still receive the $150 gift card upon completion of the program steps in 2018 (program requirements must again be completed in 2019 to receive the 2019 incentive reward).
Incentive Program Requirements
Your Three Steps:
- STEP 1: Submit your 2018 health screening results to StayWell.**
You may complete your health screening by attending an on-site event at UWM, or at another facility, or by submitting the Health Care Provider Form. (see below)
On-site health screenings at UWM:
- July 25, 2018, Union Ballroom, 8:00 A.M.–12:00 P.M.
- August 7, 2018, Union Ballroom, 8:00 A.M.–3:00 P.M.
- Watch for additional dates to come.
A health screening takes approximately 20 minutes to complete, and includes measurements for Blood Pressure, Body Mass Index (BMI), Cholesterol HDL, LDL and Total, Triglycerides, and Glucose. Fasting is not required, but recommended for more accurate results. Screening completion will be securely uploaded to the StayWell wellness portal within two business days. Your results will be automatically uploaded to your personal account profile within 10 business days.
Please Note: Registration is required to secure your on-site screening appointment time. Log in to your StayWell Health Portal to register for your appointment.
Alternative to the On-site Screening
As part of your routine annual preventive exam, you may have your physician complete StayWell’s health screening form, and submit this directly to StayWell before the deadline to complete your Health Screening requirement. See your StayWell Health Portal for the form and more information.
