Ready to Enroll?

When you have fully prepared and made decisions regarding your benefits, you can make your elections using Self Service.

You will be required to provide the following information in Self Service, so make sure to have this information handy:

  • Dependent names, Social Security numbers and dates of birth
  • Copy of health insurance card (if you have other health insurance coverage)

All benefit elections made via Self Service must be submitted by 4:30 P.M., on Friday, October 26, 2018.

Paper applications must be received by your Human Resources office by 4:30 P.M. on Friday, October 26, 2018. If mailing, applications must be postmarked by Friday, October 26, 2018.

Changes are effective January 1, 2019.
