Complete this form to request a Withdrawal from a semester after the published deadline. Enter a statement of rationale addressing what GER you are requesting to waive and why you feel you meet that requirement through other work/courses.
Upload the following items before you submit your request:
- Documentation that supports a waiver
Upon receipt the appeals committee will review and if approved, your advisor will submit the appropriate paperwork to the Registrar’s Office for processing of the withdrawal. The appeal decision will be communicated to you via your UWM email address.
