Written by C21 Graduate Fellow Azin Naderi.

Losing weight and reaching a stereotypical fit body is something that a wide range of people, from those with morbid obesity to completely average weight, are always looking for. The U.S. Centers for Disease Control and Prevention (CDC) website notes that, for adults, obesity has three ranges, starting at a BMI of 30 or higher (203 pounds or more for someone 5 feet 9 inches tall), regardless of age, sex, or race. In this post, I’m NOT talking about obesity in any range; it’s all about individuals with a BMI of less than 30.

Shortcut No. 1: Under the Knife

Weight-loss surgeries started around 70 years ago with jejunoileal bypass. Diarrhea, malnutrition, and liver failure were the reasons for the discontinuation of this surgery. Stomach stapling was designed in the early 1970s, and one version of it was once very popular, but it fell out of favor because of weight regain and severe heartburn. Gastric bypass, which was developed in the 1960s, had become the most common weight-loss surgery in the United States by the mid-2000s, with about 140,000 operations projected for 2005. It remains popular today. In 2011, adjustable gastric bands (such as the Lap-Band) accounted for about one-third of all such procedures, even though some studies had already found that a substantial number of patients needed another operation because of long-term complications; they have since virtually disappeared. Since 2013, sleeve gastrectomy has emerged as a leading procedure and remains the most prefered option.

Other procedures are not meant to be a solution for obesity; they are intended for people who are close to a normal weight, and they remove fat from specific areas of the body or, in the case of the “Brazilian butt lift (BBL)”, move it to another part of the body. Liposuction emerged in the mid-1970s, arrived in the United States in the early 1980s, and by the early 2000s was the country’s most common elective cosmetic procedure. A survey of plastic surgeons covering 1994 to mid-1998 found that approximately one in 5,000 patients died from the procedure; a later survey found a lower death rate, which was attributed to changes in how physicians practiced. The BBL, which involves injecting fat into the buttocks, carries more risk than most other plastic surgery procedures: its death rate was once estimated to be as high as one in 3,000, which led surgeons to change the technique.

Any solution in these two categories is “surgery” in the end, but surgery comes with a cost. The post-surgery recovery period can be lengthy and painful, and a weight-loss surgery can cost between $15,000 and $25,000 or more. So, what’s the solution?

Shortcut No. 2: The Needle

The now-ubiquitous weight-loss injections come from one medication family: GLP-1, which includes trade names like “Ozempic”, “Wegovy”, “Mounjaro”, and “Zepbound”. Semaglutide was first approved in 2017 for diabetes under the trade name “Ozempic”, and in 2021 a higher-dose version was approved for weight loss under the trade name “Wegovy”. By late 2025, about 12 percent of adult Americans said they were using a GLP-1 drug for weight loss or for a condition like diabetes, double the share 18 months earlier. Wegovy is approved for adults with a BMI of 30 or more, or 27 with at least one related disease, such as high blood pressure or diabetes. For example, someone 5 feet 7 inches tall who weighs about 192 pounds or more, or about 173 pounds or more with one related disease.

Official list prices average about $1,000 a month. In late 2025, deals were announced with the makers to offer lower prices directly to consumers, but the cost continues until the end of use, and more than half of users say the drugs were difficult to afford. In its largest trial, Wegovy reduced average weight by about 12% more than a placebo over 68 weeks (roughly 16 months), about 25 pounds for a 200-pound person. The most common side effects include nausea, diarrhea, vomiting, and constipation. The drug’s label carries warnings about pancreatitis and gallstones, as well as a boxed warning regarding a potential risk of thyroid tumors. However, the biggest issue is weight regain: a large proportion of people stop taking the medication within a year, and after stopping, they regain about two-thirds of the lost weight within the first year and are back at their starting weight within about 18 months. The question here is: why do people without obesity do this to their bodies? In my opinion, the answer is simple: impatience with SLOW progress!

No Shortcut: My Slow Story

I’m not the kind of person who acts like an armchair quarterback, so I want to share my personal story. For different reasons, I gained around 33 pounds in one year, and for the first time in my life, I became overweight with a BMI of 27. I wasn’t happy with the impact the weight gain had on my day-to-day life, for example, I could no longer fit in any my clothes. I knew I wanted one thing: to lose this weight as soon as possible! I tried different things, from fasting to doing at-home exercises and eating smaller portions. But they were all temporary, and I got stuck in a loop: I didn’t lose much in a short time, I got disappointed, and I quit the method and gained weight again! I was in this loop for several months, and then I started to convince myself that I couldn’t reach my goal weight in a short time, and that I shouldn’t expect a very different result every day on the scale! This first step was as hard as the whole process, because you feel the pressure, you become hungry, and you want to lose that fat as soon as possible, yet the scale just doesn’t seem to cooperate!

Changing all my eating habits and counting calories wasn’t easy in the first few weeks. The scale wasn’t generous. I really wanted to break the cycle and finally do something effective! At the same time, my friends nagged about my diet: “Ugh…you and your diet! Let’s get a pizza, a bottle of beer, an ice cream…” But for me, the process felt very slow, and I didn’t want to do anything that would make it even slower! After two months, I could see some changes, and I started to believe in the process and in being patient. Finally, after 5 months, I lost 34 pounds, and my BMI dropped to 21. I visited my doctor, had a blood test, and the results confirmed that the slowness finally worked! Not in a harmful way, but in the healthiest way!

Reaching my goal weight without any side effects from surgery or injections was one side of this slow process; the other was the habits I built during this time: eating healthier and trusting the slow process, not just for losing weight, but for other aspects of my life. Now, 6 months later, I’ve gone back to eating sugar, carbs, oil, and alcohol sometimes without any hesitation, and still my weight remains the same. I still have a long way to go before I can call myself patient, but I’m much more patient than I was last year.

Achieving a goal in a short time feels great; it feels better than being disappointed and brings more joy, but what if the price is your health, which could have serious effects?

All Slow Digest posts are independently researched and written by individual contributors.
