Roster Addition/Removal Form

  • This form should be used for any roster additions or subtractions following the initial submission of your club's official roster. Roster additions must be done at least 3 business days prior to an added club member’s participation in a practice, and 10 business days prior to participation in a competition. Added members must also have a signed copy of both the Sport Club's Participant Agreement and Waiver on file with UREC before any club participation will be allowed.
