CONFIRMATION NEWSLETTER AND PARENTAL CONSENT FORM

Confirmation packets are sent via email, along with your registration receipt. If you do not provide an email address during registration, packets are mailed to your home address. The links below provide the same information in a downloadable PDF.

To simplify the process, we have combined the Behavior Contract and Parental Consent forms into one document. Please complete and return the Parental Consent form for each child enrolled in our classes, and review the Confirmation Newsletter for information on check-in, safety, parking and more.

DPI SCHOLARSHIP INFORMATION SHEET, DPI SCHOLARSHIP APPLICATION FORM & NEEDS-BASED SCHOLARSHIP APPLICATION FORM

Scholarship applications are accepted beginning Jan. 1 of each year. CFK/CFT offers two types of scholarships:

• Department of Public Instruction (DPI) – Do not fax this form; we need the original form.
• DPI Scholarship Information Sheet
• DPI Scholarship Application Form

Requirements: a child must be entering sixth grade or above in the fall, must have a 2.5 GPA or greater, and must be eligible for free or reduced school lunch program. The application form must be certified by your school counselor/principal. In addition, a transcript of the child’s grades is required.

• Needs-Based (Please use this form only if the child will be in 5th grade or lower next fall.)
• Needs-Based Application

Requirements: a child must qualify for free or reduced school lunch program. The application form must be completed and signed by your school principal. Students are responsible for paying $15 of the course fee. This scholarship is good toward classes with fees of $169 or less. You must pay the difference for anything exceeding $169.

Your packet is complete when all forms are filled out correctly and all documents are sent in the mail. Please do not fax in DPI applications; we need the original paperwork.

There is limited funding, so send your scholarship packet early. Needs-based scholarship recipients are notified the first week of May. Others are notified as soon as we are able to review them. Selected recipients are notified by phone or by mail.
