Employees and spouses enrolled in the state group health insurance program can earn a $150 Well Wisconsin incentive for completing a free health screening and an online assessment by Oct. 20, 2017.
The 20-minute screening will measure blood pressure, body mass index, cholesterol, triglycerides and glucose. Fasting is recommended for more accurate results but is not required. UWM will host four screenings:
- July 31, 8 a.m.-3:45 p.m. in the UWM Student Union ballroom
- Aug. 24, 8-11:45 a.m. in UWM School of Continuing Education Room 7480, 161 W Wisconsin Ave, Milwaukee, WI 53203
- Sep. 12, 8 a.m.-3:45 p.m. in the UWM Student Union ballroom
- Sep. 21, 8 a.m.-3:45 p.m. in the UWM Student Union ballroom
Participants should register for a screening at the StayWell account portal and then complete a 10-minute, mobile- and tablet-friendly health assessment at the portal.
