Earn $150 Well Wisconsin incentive with health screening

As a part of the state’s Well Wisconsin program, employees and their spouses/domestic partners who are enrolled in the State of Wisconsin group health insurance program are eligible to earn the $150 Well Wisconsin incentive by completing three steps by Friday, Oct. 19.

Step 1: Get your 2018 health screening. Log in to the StayWell portal at wellwisconsin.staywell.com. Then click the programs tab to locate and register for a free Well Wisconsin health screening event.

UWM’s on-site screening events for summer 2018 are as follows:

  • Monday, Sept. 17, 8 a.m.–noon, UWM Student Union Wisconsin Room
  • Monday, Sept. 24, 8 a.m.–noon, UWM Student Union Wisconsin Room

Screenings take about 20 minutes and include measurements for blood pressure, body mass index, cholesterol (HDL, LDL and total), triglycerides and glucose. Fasting is not required but recommended for more accurate results. On-site health screenings are free.

Screening completion will be securely uploaded to the StayWell wellness portal within two business days.  Your results will be automatically uploaded to your personal account profile within 10 business days. Individual health information will not be shared with the employer.

Alternatively, you may have your physician complete StayWell’s health screening form and submit it to StayWell before the deadline. See the StayWell site for the form and more information.

Step 2: Complete your 2018 health assessment on the StayWell account portal. It takes about 10 minutes and is mobile and tablet friendly.

Step 3: (NEW) Complete one of up to seven available wellbeing activities through the StayWell account portal. Visit the wellbeing activity button or the programs tab for more information and to select an activity.
