New Wellness and Incentive Program for 2017

StayWell is the State of Wisconsin’s new Well Wisconsin program administrator for 2017. Well Wisconsin offers tools, webinars, and resources to help you achieve your health goals.

2017 Incentive Program Details

To get started, employees, retirees, and their spouses/domestic partners,1,2 who are enrolled in a Wisconsin State Group Health Insurance Plan may complete the StayWell health screening and health assessment,3 by October 20, 2017, to earn a $150 Well Wisconsin incentive.4

New Web Health Portal

Log in to complete the steps mentioned below in the secure StayWell Health Portal at: wellwisconsin.staywell.com. Call the StayWell help line at 1-800-821-6591 if you need assistance accessing the health portal.

Two Easy Steps

Just follow the step-by-step instructions on StayWell’s Health Portal to:

  1. Get your health screening, and submit your results to StayWell.
  2. Take the StayWell online health assessment. It is mobile friendly and only takes about 10 minutes to complete.

New Incentive Deadline

In order to earn the 2017 incentive, both steps must be completed and submitted to StayWell by October 20, 2017.

New Instant Incentive Available

Redeem your $150 incentive instantly using the StayWell portal or request to receive your Visa gift card by mail once you have completed both steps.

Health Screening Dates

From your health portal, one of your screening options is to locate and register for a free StayWell on-site health screening, offered at various state employer locations and dates throughout the year.

Quest Diagnostics provides these on-site screening services. The health screening measures your blood pressure, body mass index, and collects a blood sample by finger-stick to measure your cholesterol, triglycerides, and glucose levels. The screening process, including your results, will take less than 20 minutes to complete; on-site screening results are automatically submitted to StayWell on your behalf, Step 1 towards earning your incentive.

Health Screening FAQ


1 Effective January 1, 2016, adult children are no longer eligible for the Well Wisconsin Program incentive (only your enrolled spouse/domestic partner). This is the result of the Equal Employment Opportunity Commission’s (EEOC) recent ruling on the applicability of Genetic Information Nondiscrimination Act (GINA) to employee wellness programs and the proposed rule amending GINA to allow spouse participation.

2 All wellness incentives paid to participants of the State of Wisconsin or Wisconsin Public Employer Group Health Insurance Programs by StayWell are considered taxable income to the group health plan subscriber and are reported to your employer for tax purposes.

3 Health Information, including responses to the health assessment, are protected by federal law and will never be shared with your employer.

4 If you are enrolled in Humana’s Medicare Advantage Plan or have a postal address outside the U.S. you are not eligible to receive the $150 incentive.
