  • When this form is submitted, each instructor listed will receive notification of your accommodation request for captioned audiovisual materials. Each notification will include the personalized information you provide in the form:


    Hello,

    I am registered with the Accessibility Resource Center (ARC) and have a Verified Individual Services Accommodation (VISA) that reflects an accommodation of captioned audiovisual materials for all of my classes.

    If you don’t already have captioned media such as pre-recorded online lectures, YouTube clips, DVD/VHS films, ARC can assist you. To request captions, please fill out the online Media Captioning Request Form. YouTube’s auto-generated captions are not an accurate or reliable substitute for regular captioning.

    Please be aware that the transcription and post-production captioning process is lengthy and ARC requires at least 15 business days to complete your request. You can generally avoid delays by selecting media that is already captioned, or build your course lectures with your own prepared transcripts whenever possible. If you have any questions related to media accessibility, please send an email to caption@uwm.edu.
